So this tee fitting has three different threads, right? Just want to make sure I'm seeing that correctly.

Yes, that's right. This tee features one 3/4-14 Male NPTF thread and two 1/2-14 Female NPSM Swivel threads.

Got a 3/4 male NPTF port on a manifold block and need to split the line out to two 1/2 inch hose ends, this tee gonna work for that kind of setup?

Yeah, that's pretty much what this fitting is built for. The run port is 3/4-14 Male NPTF, so it threads straight into a female NPTF port on your manifold. The two branch ports are both 1/2-14 Female NPSM Swivel, which mate up with male NPSM hose end fittings. The swivel feature on both branch ports lets you rotate each hose into position after the tee is seated, which saves a lot of grief during final assembly when clearance is tight.
