trying to hook a 3/8-18 female nptf port to a 3/8-18 npsm swivel hose end, is that what this adapter is for?

For tying those two thread styles together, this straight adapter is set up with a 3/8-18 female NPTF on one side (SAE J476) and a 3/8-18 female NPSM swivel on the other side (SAE J514).

rated for 6,000 psi -- that hold up on continuous hydraulic lines or just rated for peaks?

6,000 psi is the maximum dynamic pressure rating, meaning it covers working pressure under active flow conditions, not just static or spike loads. For a straight steel adapter running on a live hydraulic line, that rating applies to the pressure the system is actually pushing during operation. As long as your system stays within that threshold, you're in good shape for continuous use.
