Reckon this'll hold up to a 6000 PSI application?

it's rated for a maximum dynamic pressure of 6000 PSI, so it should handle that just fine.

Got a line running 1/8-27 NPSM on one end and need to tie into a 1/4-18 NPTF port on the other, this fitting do that?

That's what this one's built for. The male end is 1/4-18 NPTF, which meets SAE J476, and the female swivel end is 1/8-27 NPSM per SAE J514. The swivel feature on the female side helps a lot when you're trying to connect without twisting the line during installation.

trying to swap a fitting on a small line, is this one 1/4 male pipe on one end and a 1/8 female swivel on the other?

On the thread side, it’s a 1/4-18 Male NPTF (SAE J476) on one end and a 1/8-27 Female NPSM swivel (SAE J514) on the other, so it’s meant to transition between those two thread types and sizes.
