trying to swap out a 90 with a 45, is this one M24 x 1.5 on both ends or just one side?

Both sides are M24 x 1.5 DIN Tube connections. Side 1 is male (S16) and side 2 is female (S16), so it is set up as an M24 x 1.5 male to M24 x 1.5 female 45 degree swivel elbow.

Both sides on this fitting are M24 x 1.5 thread, does one end screw into a fixed port and the other spin free, or how's the swivel part work exactly?

One side is male and one side is female, both carrying that M24 x 1.5 DIN tube thread. The swivel function lets the female end rotate independently once the fitting is assembled into your line, which keeps hose or tube from twisting under pressure or during routing. That rotation is what makes it useful when your connection points don't line up cleanly and you need some give without stressing the joint.
