what all sizes does this mounting flange come in, and will it work on a 54H or 64H pump?

Both the 54H and 64H configurations are covered by this flange. The package itself is listed as a mount flange sub-assembly for exactly those two pump sizes, and Eaton makes it available across all configurations and sizes in that line. If you're running either of those piston pump setups, this is the right accessory to have on hand.
